About Rise Assessments

  • Children experience learning through their sensory systems. When sensory input feels overwhelming, under-responsive, or hard to organize, it can affect attention, behavior, emotional regulation, and participation in educational activities.

    The Rise Forward Sensory Needs Assessment is an educationally focused evaluation designed to help families better understand how a child’s sensory processing impacts their learning environment and daily routines.

    This assessment provides guidance and recommendations to support success in home-based, homeschool, and educational settings.

  • Rise Assessments are educational in nature and are not medical or therapeutic services.

    They do not diagnose conditions, provide treatment, or replace occupational therapy, medical care, or other clinical services. Instead, they focus on understanding patterns that influence learning and daily participation and offering recommendations to support those areas.

    This one-time assessment helps families understand how sensory processing influences attention, behavior, and engagement in educational settings. Recommendations are designed to be practical, realistic, and easy to implement at home or in learning environments.

    Includes:

    • Caregiver interview

    • Standardized sensory questionnaire

    • Professional review and interpretation

    • Written educational summary and recommendations

  • Rise Assessments are well suited for families who:

    • Homeschool or use alternative educational models

    • Utilize ESA funding or private pay

    • Have children who are autistic, ADHD, or neurodivergent

    • Want guidance selecting sensory tools and environmental supports

    • Are seeking clarity without initiating therapy services

  • Assessments are scheduled in advance and completed through a parent-focused interview, either virtually or in person. Intake forms are completed prior to the appointment, and families receive a written summary with recommendations after the assessment is complete.

    Sensory Needs Assessment: $350

    Family rates may be available for multiple children.

    Payment is required at the time of booking.
